abstract = {Population assays and analyses of sea water were conducted simultaneously at various places of French Polynesia to investigate environmental factors affecting the distribution and population of Gambierdiscus toxicus. The population of the organism was low at Tahiti, Moorea and Bora Bora islands but was extremely high at the Gambier islands. Thus the ciguateric endemicity of the surveyed area was well explainable by the abundance of the organism. Nutrients such as the inorganic phosphorus, total phosphorus, nitrite, nitrate, silicate, iron, dissolved organic carbon, and vitamin B12 showed little correlationwith the population of G. toxicus. The benthic community consisting of corals, macroalgae and benthic microalgae such as diatoms and dinoflagellates seemed to play an important role in regulating the population of the toxic dinoflagellate.},

abstract = {Pour essayer de mettre au point un protocole thérapeutique tenant compte au mieux du mode d'action des toxines impliquées dans la pathogénie ciguatérique, les auteurs ont étudié, chez la souris, les effets sur le temps létal de l'administration conjointe d'extraits toxiques et de certaines drogues à vocation protectrice. Dans les conditions expérimentales, les résultats les plus significatifs au plan des applications pratiques sont obtenus avec le glutamate de calcium associé à la vitamine B12.The authors try to find a therapeutic protocol taking in account for the best mode of action of the toxins involved in the ciguatera pathogeny. For this purpose, they have studied by mice the effects on the lethal time of the joint injection of toxic extracts and of some a priori protecting drugs. In the experimental conditions, the most significant results for the practical applications are obtained with calcium glutamate associated with B12 vitamin.},

abstract = {On suppose que la mortalité due au parasitisme ne se produit pas abruptement au-delà d'une certaine charge parasitaire limite, mais que la survie diminue progressivement en fonction du parasitisme, suivant une exponentielle décroissante. Dans ces conditions, si la distribution des parasites chez leurs hôtes suit une loi binomiale négative dont l'exposant k, mesurant la surdispersion, caractérise le système considéré, on constate que ce modèle de pathogénicité parasitaire ne modifie pas le type de distribution, qui reste une binomiale négative, ni sa surdispersion. Ces modèles sont confrontés à des résultats expérimentaux et à des observations dans les conditions naturelles, portant sur divers couples hôtes-parasites. Ils permettent, dans certaines conditions, d'estimer un paramètre épidémiologique primordial, le coefficient de pathogénicité parasitaire.Vectorial mortality due to parasitism is supposed not to occur sharply when some parasitic densitythreshold has been raised, but to be a progressive phenomenon, survival decreasing exponentially with the parasitic burden. The distribution of parasites among hosts is supposed to follow a Negative Binomial Law, the k exponent of which, measuring overdispersion, characterizes the system. One finds that this model of parasite pathogenicity does not modify either the type of the distribution, which remains a Negative Binomial, or its overdispersion. These models are confronted with experimental results, and with field observations. In some conditions, they allow to estimate the parasitic pathogenicity coefficient, a basic epidemiological parameter.},

abstract = {Au centre de transfusion de Papeete (île de Tahiti), l'antigène HBs a été recherché chez 5406 donneurs de sang par la méthode de l'hémagglutination inverse : 2,53 p. 100 d'entre eux étaient porteurs. Chez les donneurs métropolitains nouvellement arrivés sur le territoire, le portage se situe à un taux de 0,27 p. 100 alors qu'il est retrouvé chez 3,01 p. 100 des donneurs autochtones. De ce point de vue, la situation de la Polynésie est meilleure que dans certains territoires qui l'entourent, mais elle est moins bonne que celle des pays développés. La Polynésie est trop vaste et trop hétérogène pour qu'on puisse extrapoler ces chiffres à l'ensemble du territoire. Mais néanmoins, la recherche de l'origine géographique des porteurs, comparée à celle des donneurs, fait apparaître que la prévalence du portage de l'antigène HBs est plus forte dans les archipels éloignés qu'à Tahiti. D'autre part, la recherche des sous-types de l'antigène HBs a permis de révéler la présence d'un nouveau sous-type adr q négatif qui semble bien particulier au Pacifique Sud. En Polynésie française, il semble localisé à certains archipels. Par ailleurs, on note la fréquence et la prépondérance du sous-type adw4 dans d'autres archipels. Ces conclusions amènent à des hypothèses d'ordre ethnologiques. Cependant, de plus amples études épidémiologiques semblent nécessaires pour préciser la fréquence du portage de l'antigène HBs et la répartition géographique de ces sous-types, dans les divers archipels du territoire.In the blood center of Papeete (Tahiti island), a survey on HBs antigen has been made in 5406 blood donors by reverse hemagglutination method. Its shown 2.53 p. 100 of carriers. In european donors recently arrived in the territory, the rate of carrier state is 0.27 p. 100 versus 3.01 in the autochtonous donors. From this point of view, the situation of Polynesia is better than that prevailing in some of the suroundings territories but worse than in developped countries. French Polynesia is too extended and too heterogenous to permit an extrapolation of data to the whole territory. Anyway, the study of the geographical origin of the carriers, if compared to that of the donors, gives evidence that the prevalence of HBs antigen carriage is higher in remote archipelagos than in Tahiti. The research of subtypes of HBs antigen shown the presence of a new subtype adr q negative which seems very particular to South Pacific. In French Polynesia, it seems localized to some archipelagos. In addition, the frequence and preponderance of adw4 subtype in other archipelagos have been compiled. The data collected lead to ethnological hypothesis. More extensive epidemiological studies appear necessary to precise the frequency of HBs antigen carriage-state and the subtypes distribution in the various archipelagos of the territory.},

abstract = {We consider the precise role, in the biogenesis of ciguatera, played by the dinoflagellate Gambierdiscus toxicus which had been previously isolated in the Gambier Islands (French Polynesia) from toxic biodetritus covering dead corals. We studied the characteristics of the toxicity and its quantitative correlation with dinoflagellate numbers, assaying various samples of non-fractionated biodetritus, a sample of biodetritus fractionated according to particles size, and samples of G. toxicus cultured cells. The toxic substances, isolated in vivo and in vitro, have almost the same biochemical and biological properties as the reference ciguateric toxins. The direct and reproducible relationship between the number of G. toxicus cells and the toxins concentration in the biodetritus, and the capacity of the monoalgal G. toxicus cultured cells to produce the ciguatera toxin complex, confirm the dinoflagellate as the responsible agent of the phenomenon in French Polynesia. The distribution of this dinoflagellate in other endemic areas of the Pacific and the West Indies, provides a presumptive argument for a common worlwide origin for ciguatera.},

abstract = {A new toxic dinoflagellate found in mixtures of algae and detritus in ciguatera-endemic areas is identified as the most likely cause of ciguatera. Samples contained both fat soluble and water soluble toxins. The former is identified as ciguatoxin, the principal toxin of ciguatera, and the latter is identical with maitotoxin which is associated with herbivorous ciguateric fishes. Specimens obtained from unialgal culture indicate high production of maitotoxin, but low amounts of the fat soluble toxin. Purified maitotoxin, but low amounts of the fat soluble toxin. Purified maitotoxin appears to be one of the most potent marine toxins known, having a lethal dose (ip to mice) of 5 mg/kg. It shows strong hemolytic and ichtyotoxic activities as well.},

abstract = {Clinical observations on ciguatera were collected between 1964 and 1977 on 3,009 patients from several South Pacific island groups. Patients generally presented with neurologic symptoms such as parasthesia, vertigo, and ataxia, in addition to gastrointestinal symptoms such as diarrhea, abdominal pain, nausea, and vomiting. Patients with this illness usually became symptomatic less than 24 hours after ingestion of the fish and most patients (76.8%) developed symptoms in less than 12 hours. Significant differences in certain symptoms were noted between Melanesian and Polynesian ethnic groups, suggesting a susceptibility difference, or a difference in the nature of the toxin found in different areas of the Pacific. Being poisoned multiple times appeared to result in a clinically more severe illness than disease observed in patients experiencing ciguatera for the first time.},

abstract = {Dans un même environnement, deux méthodes de capture de moustiques, l'une sur appât humain, l'autre au piège lumineux, montrent d'évidentes différences dans la diversité et l'abondance des espèces : le piège lumineux récolte de nombreuses espèces mais relativement peu de spécimens, alors que les récoltes sur appât humain procurent de nombreux spécimens n'appartenant qu'à peu d'espèces.Les échantillons de moustiques anthropophiles s'ajustent mieux à une distribution lognormale tronquée qu'à la série logarithmique de Fisher. Les courbes théoriques de Gauss ajustées aux échantillons anthropophiles et phototropes diffèrent d'une façon significative en variance mais montrent la même richesse (c'est-à-dire le même nombre total d'espèces), suggérant que les deux échantillons proviennent en fait de la même communauté d'espèces.
La richesse estimée de cette nomocénose est en concordance avec l'inventaire faunistique établi par les entomologistes médicaux qui se sont succédé dans cette région depuis trente ans.

The complete theoretical Gaussian curves fitted to the anthropophilic and phototropic samples significantly differ in variance, but apparently show the same richness (i.e. the same total number of species), suggesting that both samples come in fact from the same species community.},

Deux méthodes de contrôle de la microfilarémie ont été utilisées : la méthode des six gouttes épaisses et celle de concentration par centrifugation du sang hémolysé. Elles ont toutes les deux montré que cette association médicamenteuse agit très vite sur la microfilarémie et que son effet persiste plusieurs mois. Cependant le nombre important de réactions d'intolérance fait que cette association n'est pas utilisable en médecine de masse.The authors tested the association diethylcarbamazine (200 mg)-levamisole (150 mg) in single dose on 25 carriers of Wuchereria bancrofti microfilaria. Two methods of microfilaremia control have been used : the method of 6 thick drops and the one of concentration by centrifugation of hemolyzed blood. Both of them showed that this two drug association acts very quickly against the microfilaremia and its effect lasts during several months. Meanwhile, the important number of intolerance reactions makes this association impossible to use in mass medicine.},

abstract = {Following the previous discovery of a new toxic dinoflagellate Gambierdiscus toxicus as the most likely cause of ciguatera, an ecological survey was undertaken on this organism around Tahiti Island and in the Gambier Islands of French Polynesia. In the survey the epiphytic nature of the dinoflagellate was disclosed and a rapid assay method was established by taking advantage of this living pattern. The method consisted of shaking off of the organisms from the appropriate substrative algae and subsequent microscopic counting of the cells. The population of the organism was found much higher in the Gambier Islands than around Tahiti, even though there were marked regionalities in both survey areas. The distribution map prepared by the survey not only agreed well with the results of the past surveys but gave a more quantitiative and clearer picture of the ciguatera-endemicity of the areas.},

La majorité s'est révélée atoxique pour la souris. Parmi les espèces actives, seule une souche de Gambierdiscus toxicus renferme des toxines très semblables sinon identiques aux toxines ciguatériques : ciguatoxine, scaritoxine et maitotoxine. En revanche cette dernière toxine apparaît plus commune et présente dans plusieurs espèces de dinoflagellés.Toxicity study of 15 strains of various algae (cyanophyta, dinoflagellates, ...) cultivated in a artificial medium was achieved. Most of them were non toxic to mice. Toxins very similar to (or identical with) ciguateric toxins (ciguatoxin, scaritoxin and maitotoxin) were found only in a strain of Gambierdiscus toxicus. On the other hand, maitotoxin seems to be more common and present in several species of dinoflagellates.},

La pose de pièges à émergence sur des trous du crabe terrestre Cardisoma carniflex montre que ce type de gîte larvaire, le seul productif hors des villages, n'est pas un gîte de repos pour ces femelles. La capture systématique de moustiques adultes au repos dans 16 maisons témoins de la zone urbaine de Papeete montre que Ae. polynesiensis est strictement exophile. Un exemple d'application pratique des conclusions de ces expériences est donné par un essai de lutte contre Ae. polynesiensis autour d'un site touristique dans l'île de Bora Bora. L'éducation sanitaire est une arme importante dans la lutte contre les Aedes (Stegomyia) exophiles.Three catches data of Aedes (Stegomyia) polynesiensis males and females are analysed. In Tuamotu islands hand net catches give an indication on the preferred vegetable association for resting females. Exit traps upon crabs holes (Cardisoma carniflex) show evidence of no resting females. In Papeete town (Tahiti) indoor catches (16 houses for two months) lead to think that Ae. polynesiensis is a strictly exophilic mosquito. An attempt to control Ae. polynesiensis in the touristic area at Bora Bora airport, by destroying all plants from 0.20 m to 4 m high, on a distance of 160 m all around the buildings, has given a large reduction of agressive mosquitoes (96%). Until the discovery of new methods for Aedes control, the sanitary education of people remains the only economic way to limit Ae. polynesiensis in South Pacific.},

En 1975, un millier de formes préimaginales de T. amboinensis sont récoltées dans 49 des 59 agglomérations de l'île de Tutuila. Des observations sur la biologie dans la nature du moustique prédateur sont notées. Du fait de conditions écologiques très comparables, T. amboinensis est ensuite élevé de façon extensive à l'insectarium de Paea à Tahiti. Depuis, environ 50.000 adultes ont été lâchés régulièrement à Tahiti, un lâcher de 100 adultes est réalisé dans l'île de Moorea, 300 à Maiao ; 4000 oeufs sont transportés à Mangareva, île principale de l'archipel des Gambier.Les études entomologiques actuelles montrent que l'espèce s'est parfaitement adaptée aux biotopes des îles hautes de Polynésie française. Un équilibre proie prédateur s'instaure rapidement. Les populations des Aedes proies restent encore suffisamment importantes pour que la transmission de la filariose et des épidémies de dengue se perpétuent. Seuls les lâchers "inondatifs" de T. amboinensis élevés en masse de façon industrielle permettront de contrôler efficacement les espèces vectrices visées.

abstract = {The author puts together the data and reports from different specialists who studied, in relation to the UNESCO MAB project "Man and Biosphere", theme 7, the terrestrial biota of the atoll of Takapoto (Tuamotu archipelago, French Polynesia). In order to describe the atoll biota, the author makes out five different habitats, and comments upon the food-cycle structures that control the relations between flora and faune. Although the determination of all the available species is not yet over as far as entomology is concerned, it is most likely that the animals of Takapoto had a "pioneer" action. This fauna settled very recently and has an universal origin. This is in perfect harmony with the theory of the re-emerging of the Tuamotu atolls.The animal and vegetal species are now spreading over Takapoto, mainly from the australian and asian regions, most often via Tahiti. However, due to the fast-developing continent to continent means of conveyance in the Pacific, the latest species that have reached the island may originate from farther countries.
But for birds perhaps, the speciation does not seem, up to now, to have developed into new species adapted to the archipelago as it is the case for neighbour high islands like Tahiti or the Marquesas.
The inhabitants of Takapoto can only live on and hold out by fulfilling their needs mainly with sea and lagoon products rather than very poor terrestria ressources. abstract = {L'étude chronobiologique du phénomène de subpériodicité chez les microfilaires de W. bancrofti var. pacifica indique qu'il n'est pas dû, comme le suggère Hawking, à une certaine 'dégénérescence' de la forme périodique nocturne, mais qu'au contraire il paraît traduire une plus grande richesse génétique, préservée par les habitudes de son vecteur principal, Aedes polynesiensis. La subpériodicité est attribuée à un mélange de microfilaires avant des biorythmes circadiens différents, certains dont la période est de 24 heures, mais dont la phase diffère, d'autres dont la période est différente. Ces derniers paraissent être remarquablement stables, puisqu'ils peuvent donner lieu à des fluctuations apparentes cycliques de basse fréquence pendant plus d'un an, par effet de pliage.A partir de ces résultats, diverses hypothèses sont émises :
- La périodicité stricte n'est pas considérée comme un mécanisme sophistiqué destiné à promouvoir la plus grande transmission possible, mais comme le résultat de la sélection, à partir d'une souche 'apériodique' (composée en fait de nombreux biorythmes autonomes asynchrones), par un vecteur ayant une courte activité nycthémérale, des biorythmes dont la période et la phase correspondent à un cycle d'agressivité.
- Même chez les parasites montrant une périodicité stricte, la subsistance d'une composante circadienne indépendante du système nycthémère est envisagée.
- L'hypothèse d'une origine polynésienne de la filaire de Bancroft, à partir d'un ancêtre austronésien proche de Brugia malayi, est confrontée aux acquisitions récentes sur l'histoire du peuplement du Pacifique.
- Enfin, on envisage que la diéthylcarbamazine serait peut-être un 'déboussoleur temporel', qui perturberait le système de repérage des microfilaires et/ou les 'indicateurs de temps' (synchroniseurs) de l'hôte vertébré.
Ces diverses spéculations montrent que le chapitre sur la périodicité des microfilaires est loin d'être clos, et qu'il mériterait d'être réexaminé à la lumière de progrès spectaculaires récents de la chronobiologie.
The weak and irregular periodicity which characterizes the microfilariae of Wuchereria bancrofti var. pacifica, is tentatively explained as resulting of the superimposition of biorythms differing in phase and in period. This suspected greater genetic diversity could have been preserved by the biting habits of the main vector, Aedes polynesiensis.
Autonomous circadian rythms seem to be extremely stable : through 'folding effect', they are supposed to produce, during more a year, apparent low frecency, cyclical fluctuations.
Following this results, several hypothesis are discussed, in particular about the anteriority of var. pacifica with regard to the nocturnally periodic, 'typical' form, and about the possible chrono-pharmacological effect of diethylcarbamazine.},

abstract = {In spite of a geomorphological facies of closed atoll, Takapoto has an ichthyological fauna relatively rich, when compared to some others French Polynesia atolls. Approximatively 170 fish species belonging to 39 families have been counted in the laggon. This fauna is generally rather poor in individuals, except for some species. Amongst them, three parrotfishes (Scarus harid, S. ghobban, S. sordidus), three mullets (Crenimugil crenilabis, Chelon vaigiensis, Mugil cephalus), three butterflyfishes (Chaetodon auriga, C. ulietensis, C. lunula), two damselfishes (Pomacentrus pavo, abudefduf coelestinus) and a remora (Echeneis naucrates) were very common. More as in the Scilly's lagoon, it has been noted the presence of a relatively large population of Lethrinus miniatus, original feature for a closed atoll. It seems that seasonal variations in the fish density and distribution can occur, changing the above pattern. There was apparently no ciguatera in the lagoon at the survey moment. On the other hand, some species fished outside the barrier reef, including primarily parrotfish, snappers, groupers and jacks, can be toxic for man, as confirmed by biochemical studies.},

Dans la nature, elle adhère directement aux thalles des algues ou aux sédiments variés colonisant les substrats coralliens morts, ou reste en suspension à leur proximité. En culture, elle s'agglutine en amas importants qui adhèrent au fond ou sur les parois des récipients. Son développement optimal est favorisé par la présence de bactéries dans le milieu ambiant.The toxin producing dinoflagellate Gambierdiscus toxicus is distinctive in its anteroposterior compressed body shape with a circular, narrow deep cingulum and a short and deeply indented sulcus. The species does not move much and has a sessile behavior. The wild cells attach directly to algae thalli or sediments covering dead coral substrates or lie suspended close by. The cultured cells are often agglutinated in large packs on the bottom or sides of the containers. Their optimal development is favoured by bacterial presence in ambient media.},

abstract = {La lutte antifilarienne est facile à schématiser : chimiothérapie de masse, lutte antivecteurs directe, action antivecteurs indirecte (modification de l'environnement). Ces actions doivent être coordonnées et orchestrées par des campagnes d'éducation sanitaire appropriées et répétées. Une action prioritaire peut être choisie en fonction des conditions épidémiologiques locales. En Polynésie française, celle-ci fut donnée à la chimiothérapie par la diéthylcarbamazine. Elle se poursuit actuellement depuis près de 25 ans. Les moyens mis en oeuvre et par conséquent les résultats varient selon les îles considérées. Cinq zones différentes sont distinguées et analysées en fonction des actions qui se sont effectivement déroulées.A campaign anti-filarian is simple to schematize : mass chemotherapy, direct campaign against the vectors and indirect antivector actions (modification od the environment). These actions should be coordinated and associated with repeated sanitory education campaigns. The priority action can be chosen as a function of the local epidemiological conditions. In French Polynesia this priority proved to be chemotherapy by diethylcarbamazine and has been carried out for nearly 25 years. The measures put into action and consequently the results vary according to the islands considered. Five different zones were distinguished and analysed according to the methods performed.},

Il existe une tendance à la neutropénie au cours des crises de lymphangite survenant sur un membre éléphantiasique. Le taux des IgE est nettement augmenté, celui des IgG l'est également mais à un moindre degré. Les valeurs moyennes sont identiques chez les 'porteurs sains de microfilaires'. Par contre, le sérodiagnostic est plus fréquemment positif chez les éléphantiasiques.

Plusieurs membres sont touchés près d'une fois sur deux. L'atteinte du scrotum est rare ; celle du sein n'est retrouvée qu'une seule fois. Chez les hommes, on relève dans 1/3 des cas une hydrocèle associée. Des antécédents de chylurie ne sont pas exceptionnels.
Twenty-five years after the fight started against Wuchereria bancrofti var. pacifica in French Polynesia and which was based upon mass chimiotherapy by diethylcarbamazine (Banocide), a survey realized in 1975 and 1976 among 274 patients with elephantiasis allows us to give precise details of the local clinical and epidemiological aspects. Since 1950 the number of clinical cases varies along with the parasitological incidence, but with a certain delay which is explained by the late and irreversible elephantiasis characteristics. The overall prevalence is actually low. However the occurrence of new cases, although very rare, is still possible. Among sick patients the lymphangitic crisis frequency whose aspect lead to evoke a bacterial participation, is low. Women are less frequently and less severely involved than men. Localisations are essentially restricted to the lower limbs. Several limbs are involved in approximately 50 0/0 of the cases. The scrotum involvement is rare, the one of the breast is found only once. Among men we can notice a 33 0/0 incidence of associated hydroceles. A past history of chyluria is not rare.},

Ces méthodes permettent de recueillir des antigènes homologues en quantité et qualité suffisantes pour l'exploration immunologique à grande échelle de la filariose de Bancroft. Les auteurs signalent des possibilités d'application à d'autres domaines de recherche : écologie et biochimie parasitaires, chimiothérapie, entomologie-épidémiologie.In order to provide antigenic material in sufficient quantities for immunological studies, the authors developed a simple technique for extracting infective stage larvae of Wuchereria bancrofti from Aedes polynesiensis raised in an insectary. Experimentally infected mosquitoes were caught and after anaesthetization were held under physiological saline solution for 12-24 hours. In this time the infective larvae emerged from the mosquitoes and could be collected. The suspension of larvae was then introduced under slight pressure into a short sealed section of rat thoracic artery in order to obtain homogeneous distribution of the larvae in the artery walls. Frozen sections of the artery were then suitable for immunofluorescence tests. For the collection of microfilariae, a modification of Sawyer & Weinstein's method was used. Blood from a highly parasitized subject was collected in a citrated bottle, and, after lysis of the red cells with saponin, was centrifuged. Treatment of the sediment with streptolysin then released the living microfilariae, which when taken up in physiological saline solution were ready for use in immunofluorescence tests.},

Par la présence d'une bande "hybride" chez les hétérozygotes, la structure moléculaire de ces isoenzymes peut être considérée comme étant dimérique.Most of Aedes polynesiensis show only one locus coding for one a-glycero-phosphate deshydrogenase band. However two other rare alleles have been noted. By the genetic study of one of them, one may conclude that the only locus coding for a-glycero-phosphate deshydrogenase in Aedes polynesiensis is triallelic, codominant and autosomic. By the presence of a "hybrid" band in the heterozygote, these isoenzymes may be considered as being dimers.},

Ils sont fondés le plus souvent sur l'emploi de la diéthylcarbamazine (DEC) dont les inconvénients au cours de l'application des schémas classiques de prescription ont fait souhaiter la découverte au moins d'un nouveau schéma ou mieux d'une nouvelle drogue. La méthode des doses espacées répond à la première préoccupation. L'expérience relatée montre en effet que des doses annuelles de DEC prescrites à un groupe de porteurs améliorent considérablement le niveau de leur microfilarémie globale et devraient mener progressivement à l'épuisement de la transmission, donc à la disparition de l'endémie. Cette technique, simple et peu onéreuse, est certainement applicable sous forme de campagnes de masse dans toutes les régions où sévit la filariose lymphatique.The methods in current use for the administration of diethylcarbamazine of lymphatic filariasis are effective against microfilaraemia but require more staff and funds for longer than most countries can afford. The authors describe their findings with regards to the effect of three doses of diethylcarbamazine given at one-year intervals to 120 microfilaria carriers : the proportion of microfilaria carriers in the group fell successively from 100% to 50%, 25% and 12%, the average density of microfilariae in the blood fell from 15 per 20 mm3 of blood to 3, 0.9, and 0.3, and the maximum microfilarial density in the blood fell from 165 to 127, 15, and 11. They conclude that an effective programme of long-term chemotherapy by means of widely spaced doses od diethylcarbamazine (DEC) is feasible and has the advantage of being simple and inexpensive.},

Bien qu'il ne soit pas connu jusqu'ici comme vecteur d'agent pathogène pour l'homme, son agressivité constitue une gêne à l'essor touristique de ces îles. La dispersion de cet insecte par le vent, la capacité d'autogénie des femelles ainsi que la parfaite adaptation des formes préimaginales à différents types de biotope, sont favorables à son extension rapide. Seul l'assèchement total et permanent de toutes les zones marécageuses serait un réel facteur limitant.From about ten years Culicoides belkini came from Fidji Islands and invaded all archipelagoes of Société, Tuamotu and Cook Islands. While it is not a vector of human diseases its high agressivity and its so painfull bites brake the touristic rise of these isalnds. Due to the autogenous capacity of females well dispersed by wind and to the quite perfect adaptative possibility of preimaginal stages, this species can easily invade each island. One of the best factor to limit such an invasion could be the permanent dry up of every swamp.},

abstract = {In order to elucidate the transmission of ciguatoxin through the food chain and thus determine the primary source of the toxin, fish and molluscs of various feeding habits were collected from three ciguatera-endemic areas of French Polynesia. The specimens included carnivores, herbivores, surface grazers, coral feeders, sediment feeders, and filter deefers. Despite such diversity in feeding habits, all the fish specimens were more or less toxic by our screening method for ciguatoxin. The molluscs were non-toxic. Subsequent tests on the diets of these fish disclosed strong toxicity in a sample consisting of algae and detritus collected from the surface of dead coral of the Gambier Islands. Microscopic observation revealed the presence of a large number of a species of dinoflagellate in this sample. Comparison of similar samples from other places indicated that the dinoflagellate might be the cause of the toxicity.},

Quoi qu'il en soit et contrairement aux autres territoires exposés, l'épidémie a été contrôlée et, étalée dans le temps, n'a pas eu de graves conséquences : ni formes hémorragiques sévères, ni paralysie de l'activité du territoire. Ce résultat a été obtenu sans nécessiter la répétition d'épandages massifs d'insecticides.},

abstract = {Experiments were designed to see wether a dinoflagellate, Diplopsalis sp. nov, found in a toxic sample of detritus collected from a ciguatera-endemic area is the cause of ciguatera. The dinoflagellate in the detritus was separated from other materials by means of sedimentation and sieving through sieves of various mesh sizes. Bioassay of the dinoflagellate-rich samples thus obtained proved that the toxicity of each sample was proportionally related to the number of the dinoflagellate cells in the sample. Subsequent extraction of the dinoflagellate sample followed by fractionation of the extracts with solvents afforded two major toxins : a diethyl ether soluble toxin and an acetone precipitable toxin. The former toxin was judged to be identical, or closely related, to ciguatoxin, a major toxin in ciguatera, on the basis of various column and thin layer chromatographic properties. A close similarity was also observed between the acetone precipitable toxin and maitotoxin, a secondary toxin isolated from ciguateric surgeonfish.Judging from these results, it was concluded that Diplopsalis sp, nov. is very likely to be the cause of ciguatera.},

En plus de l'intérêt que cela comporte du point de vue fondamental, cette connaissance est primordiale pour les personnes chargées de la lutte antifilarienne. Dans de nombreuses régions, cette lutte antifilarienne est fondée essentiellement sur la chimioprophylaxie de masse (20). Le but de celle-ci est de réduire au maximum le réservoir de parasite constitué par l'homme porteur de microfilaires dans le sang circulant. Or, il n'existe toujours pas à l'heure actuelle de produit pouvant être utilisé à large échelle sans inconvénients majeurs qui soit capable de détruire les filaires adultes. De ce fait, afin de négativer définitivement le réservoir de parasite, il est nécessaire de prolonger fort longtemps les traitements médicamenteux périodiques.La longévité des filaires adultes varie selon les auteurs et il est donc difficile de se faire une opinion. Cependant le chiffre le plus communément admis est de 15 ans. Il est évident que lorsqu'on soustrait un patient à la contamination, l'extinction de la microfilarémie dépend de la longévité du parasite. A notre connaissance la valeur maxima connue concernant la persistance de la microfilarémie est de l'ordre d'une dizaine d'années. C'est pourquoi, il nous paraît intéressant de rapporter une observation faisant la preuve d'une longévité bien supérieure : une filaire Wuchereria bancrofti variété pacifica toujours féconde après 40 ans au moins d'existence.},

abstract = {Clinical and epidemiological observations suggested that a complex toxic molecule is involved in the parrotfish flesh (Scarus gibbus) poisoning from Gambier Islands. The fat soluble extract obtained from the muscles upon ciguatoxin preparation showed two toxic substances after fractionation by DEAE cellulose column chromatography. The major toxin is different from ciguatoxin judging by its chromatographic behaviour. The other is closely similar to (or identical with) ciguatoxin from the moray eel Gymnothorax javanicus. They were named SG1 for the new toxin and SG2 for the ciguatoxin like compound. Successive filtrations on Sephadex LH-20 of SG1 and SG2 gave respectively a lethality to mice of 0.03 microgram/g and 0.06 microgram/g. SG1, specifically occurs in the muscles of the parrotfish family (scaritoxin) while it is absent from other ciguateric fishes. According to that specificity and the lack of SG1 in S. gibbus liver and gut contents, the origin of scaritoxin is briefly discussed.},

En octobre 1975 aucune épidémie de dengue type 1 n'a encore été enregistrée après 33 semaines de surveillance.On March the 1st, 1975 it was known that a dengue fever outbreak (type 1) was reaching some South Pacific territories. The committee of control of dengye decided on an anti dengue fever campaign. A technical committee had to start and bring off this campaign. Technical actions (epidemiologic, serologic, virologic and entomologic surveys joined to limited insecticid sprays) came with a sanitary education campaign whose object was to inform the population and bring the people to participate effectively to the anti-larva campaign. After a survey on 33 weeks, no dengue fever outbreak type 1 was noticed.},

This observation adds support to the hypothesis according to which the very low density microfilaraemia play a role of some importance in the transmission of lymphatic filariasis and furthermore represents an obstacle for the eradication of the disease by chemotherapy with diethylcarbamazine.},

To reach an effective eradication it is necessary to control the mosquito population. Sanitary education of the human population and biological control seem to be the best ways.

abstract = {Le levamisole a été testé à la dose de 6 mg/kg répétée trois jours de suite sur dix porteurs de microfilaires de Wuchereria bancrofti dépistés à Tahiti. Aucune action macrofilaricide n'a été observée. L'action microfilaricide immédiate est au moins égale à celle de la diéthylcarbamazine. Mais la chute de la microfilarémie est moins durable avec le levamisole qu'avec la diéthylcarbamazine employée pourtant à dose plus faible. Enfin le pourcentage de réactions au traitement par levalisole est élevé.A 6 mg./kg. daily dose of levamisole was given during three days to ten carriers of microfilarias Wuchereria bancrofti in Tahiti. No action against adult worms was noticed. The immediate microfilaricid action was at least equivalent to the action of DEC. However the decrease of microfilaremia was not so long with levamisole than with DEC given to a lower dose. Lastly the amount of reactions after a treatment by levamisole is notable.},

Une étudecomparative permet de penser qu'il existe une certaine corrélation entre l'apparition de formes cliniques graves et l'évolution explosive d'une épidémie.On March the Ist, 1975, Tahiti was informed that a new dengue fever outbreak (type 1) was reaching some South Pacific Territories. A technical committee had to start and bring an anti dengue fever campaign. Technical actions (epidemiologic, viro-serologic, and entomologic surveys joined to limited insecticid sprays) came an intensive sanitary education campaign. The authors analyse the sero epidemiologic results after a survey on 60 weeks. Following a comparative study, gravity of symptoms, particularly dengue hemorrhagic fever and shock syndrome, seem to be related with the explosive evolution of an outbreak.},

abstract = {Prior laboratory studies and field observations suggested that it might be possible to reduce the size of the population of, or eliminate, Aedes polynesiensis by the introduction of Aedes albopictus. The former mosquito is the principal vector of non periodic filariasis caused by Wuchereria bancrofti and the latter is a closely related species refractory to the development of human filariae. The practicability of such competitive displacement was studied by a trial on a remote coral atoll where there was an established population of A. polynesiensis. Three strains of A. albopictus were liberated at separate localities on the atoll and their fate was followed for 4 years. One strain disappeared within 12 months after release and the other two disappeared within 48 months. It was not clear wether A. albopictus failed to become established because the strains were unsuitable, the general environment was inappropriate, or A. polynesiensis was present in such numbers that A. albopictus rarely succeeded in mating with its own species.},

Cependant on doit mettre à son actif un net recul de l'âge d'apparition de la maladie.In French Polynesia the cases of leprosy are individually severe, like in Asia, but the endemy always stood under a relatively low level. A strong campaign of chemiotherapy has been conducted for 25 years. Unfortunatly the actions of systematic detection of new cases are not sufficient and the sanitary education is almost non existent. The territory is actualy reached by deep demographic and social upsetting and chemiotherapy used alone is not enough efficient to obtain a definitive decrease of the endemy or even to avoid, for a long time, a new increase. However a significant decrease of the age of advent of the decrease must be credited to the action of chemiotherapy.},

Un tel traitement n'affecte ni l'activité physiologique chez le chat et la souris, ni le comportement chromatographique en couche mince préparative dans un système benzène-butanol (75 : 25) des molécules ciguatoxiques.A modification of the extraction procedure for obtaining ciguatoxic substances from the tissues of various poisonous reef fish has been developed. Rapid alkali treatment of the residue obtained at an early stage is efficient in separating the ciguatoxic fraction from toxic impurities which interfere with pharmacological tests. It is thus possibles to carry out a more precise dosage of the material separated in this way. Alkali treatment does not affect either the physiological activity in cats and mice of the ciguatoxic substances nor their behaviour in a preparative thin layer chromatography in a benzene-butanol system.},

abstract = {Specimens of surgeonfish Ctenochaetus striatus collected in Tahiti contained a water-soluble toxin in addition to ciguatoxin. The water-soluble toxin was tentatively named maitotoxin (MT) after the tahitian name for surgeonfish maito. Purification of MT was achievable by the standard purification procedures for polar lipids. The toxin was eluted from a silicic acid column chloroform-methanol (6 : 4) and from a cellulose column with chloroform-methanol-water (5 : 15 : 1). Upon gel-filtration through Sephadex G-25, it appeared in the fractions near void volume. Acid hydrolysis of the toxin afforded fatty acids, glucose and galactose, and 15 amino acids. The minimum lethal dose to mice by ip injection was estimated to be 15 ~ 20 mg/kg. Guppies put in 40 ppm solution of MT died within 150 min. These chemical and physiological properties indicated a close similarity of MT to the ichthyotoxin produced by the phytoflagellate Prymnesium parvum.},

En fonction des informations recueillies, ils décèlent la présence de ciguatoxine dans des poissons comestibles appartenant à des espèces à potentiel ciguatérigène latent connu, capturés dans des zones indemnes de toxicité. La ciguatoxine se présente ainsi comme une biotoxine naturelle produite en permanence au sein de la plupart des écosystèmes récifo-lagunaires polynésiens. L'apparition de manifestations cliniques chez l'homme et l'animal résulte de l'augmentation soudaine de sa production à la suite de modifications biologiques transitoires au niveau de certains substrats coralliens.Referring to the various human and animal clinical response to consumption of reef fish, the authors precise the importance of ingested dose and the role of cumulative effects. They point out the arbitrary character of distinguishing poisonous and edible fish of the same species. At the light of these data they find ciguatoxin in edible fish from known latent ciguateric potential species in atoxic areas. Therefore ciguatoxin does appear as a natural biotoxin permanently produced in most of the polynesian coral reefs ecosystems. The occurence of clinical disorders by men and animals results from a sudden increase of its production following biological transitory modification of some coral reef biota.},

There was practically no unpleasant side effect. We noticed a delayed but notable effect on microfilaremia sensibly lower than diethylcarbamazine action in similar conditions. It would be interesting to try heavier doses or iterative doses.},

Cette observation renforce l'hypothèse de l'origine écologique des toxines au long de la chaîne alimentaire de la biocoénose récifale.The use of histo-enzymological techniques for visualizing the cholinesterasic spots on the motory plate is turned to account by the authors to point out the inhibition of the cholinesterasic processes, by some fractions of the ciguatoxins at the level of the neuro-mucular synapsis. The results obtained confirm, on one hand, the efficiency of the method to test the anti-cholinesterasic activity of some fractions of the ciguateric extracts, and, on the other hand, enable the establishment of the fact that even clinically non toxic individuals from fishspecies, dwelling in tropical coral biotops, can provoke a slightly marked but real inhibition of the cholinesterases, provided the contact between the fish extract and the neuro-motory plate is long enough in time. This observation reinforces the thesis of the ecological origin of the toxins along the alimentary chain of the reef biocoenosis.},

Les conditions rarement réalisées de consommation favorable, ne provoquent pas d'empoisonnement par les poissons. Bien que les statistiques soient assez aléatoires dans les parages polynésiens, les phénomènes paraissent évoluer d'Est en Ouest, cycliquement en phase régressive fin 1973.Deux syndromes complémentaires ont été isolés :
- L'un, primaire et neuro-sensoriel, prédomine lors de la consommation de crustacés, mollusques ou échinidés toxiques. Les signes hémorragiques sont parfois observés à l'occasion de la consommation de poissons qui provoquent une toxi-infection habituelle. La spécificité des espèces toxiques n'est pas apparue à l'évidence.
- L'autre, secondaire, présente les caractéristiques myalgies transitoires et le tableau curieux d'allure névropathique de la 'contamine' de Ua Pou.
Intoxications due to sea animals in the Marquesas seem to be relatively frequent in this archipelago. Fish consumption under rarely realized favorable conditions does not induce intoxications. abstract = {To provide additional information on differences in the prevalences of Toxoplasma antibody among certain ethnic groups, particularly in relation to the relatively low prevalences noted in Chinese in Southeast Asia, Toxoplasma antibody prevalence was compared in Chinese and Tahitians living in Tahiti, where the prevalence of Toxoplasma antibody in Tahitians was known to be high. Of 23 Tahitians and 21 Chinese (mostly young adults) tested, Toxoplasma dye-test antibody (³ 1 : 16) was detected in 70% and 86%, respectively. The reason for the relatively high prevalence of Toxoplasma antibody in Chinese in Tahiti compared to Chinese in Southeast Asia was not determined.},
