The population of the organism was low at Tahiti, Moorea and Bora Bora islands but was extremely high at the Gambier islands. Thus the ciguateric endemicity of the surveyed area was well explainable by the abundance of the organism. Nutrients such as the inorganic phosphorus, total phosphorus, nitrite, nitrate, silicate, iron, dissolved organic carbon, and vitamin B12 showed little correlationwith the population of G. toxicus. De ce point de vue, la situation de la Polynésie est meilleure que dans certains territoires qui l'entourent, mais elle est moins bonne que celle des pays développés. La Polynésie est trop vaste et trop hétérogène pour qu'on puisse extrapoler ces chiffres à l'ensemble du territoire. Mais néanmoins, la recherche de l'origine géographique des porteurs, comparée à celle des donneurs, fait apparaître que la prévalence du portage de l'antigène HBs est plus forte dans les archipels éloignés qu'à Tahiti. D'autre part, la recherche des sous-types de l'antigène HBs a permis de révéler la présence d'un nouveau sous-type adr q négatif qui semble bien particulier au Pacifique Sud. En Polynésie française, il semble localisé à certains archipels. Par ailleurs, on note la fréquence et la prépondérance du sous-type adw4 dans d'autres archipels. Ces conclusions amènent à des hypothèses d'ordre ethnologiques. Cependant, de plus amples études épidémiologiques semblent nécessaires pour préciser la fréquence du portage de l'antigène HBs et la répartition géographique de ces sous-types, dans les divers archipels du territoire.In the blood center of Papeete (Tahiti island), a survey on HBs antigen has been made in 5406 blood donors by reverse hemagglutination method. French Polynesia is too extended and too heterogenous to permit an extrapolation of data to the whole territory. Anyway, the study of the geographical origin of the carriers, if compared to that of the donors, gives evidence that the prevalence of HBs antigen carriage is higher in remote archipelagos than in Tahiti. The research of subtypes of HBs antigen shown the presence of a new subtype adr q negative which seems very particular to South Pacific. In French Polynesia, it seems localized to some archipelagos. In addition, the frequence and preponderance of adw4 subtype in other archipelagos have been compiled. The data collected lead to ethnological hypothesis. More extensive epidemiological studies appear necessary to precise the frequency of HBs antigen carriage-state and the subtypes distribution in the various archipelagos of the territory.},

Patients generally presented with neurologic symptoms such as parasthesia, vertigo, and ataxia, in addition to gastrointestinal symptoms such as diarrhea, abdominal pain, nausea, and vomiting. Patients with this illness usually became symptomatic less than 24 hours after ingestion of the fish and most patients (76.8%) developed symptoms in less than 12 hours. Significant differences in certain symptoms were noted between Melanesian and Polynesian ethnic groups, suggesting a susceptibility difference, or a difference in the nature of the toxin found in different areas of the Pacific. Significant differences in certain symptoms were noted between Melanesian and Polynesian ethnic groups, suggesting a susceptibility difference, or a difference in the nature of the toxin found in different areas of the Pacific. Being poisoned multiple times appeared to result in a clinically more severe illness than disease observed in patients experiencing ciguatera for the first time. |

Until the discovery of new methods for Aedes control, the sanitary education of people remains the only economic way to limit Ae. polynesiensis in South Pacific.},

La subpériodicité est attribuée à un mélange de microfilaires avant des biorythmes circadiens différents, certains dont la période est de 24 heures, mais dont la phase diffère, d'autres dont la période est différente. Ces derniers paraissent être remarquablement stables, puisqu'ils peuvent donner lieu à des fluctuations apparentes cycliques de basse fréquence pendant plus d'un an, par effet de pliage.A partir de ces résultats, diverses hypothèses sont émises :
- La périodicité stricte n'est pas considérée comme un mécanisme sophistiqué destiné à promouvoir la plus grande transmission possible, mais comme le résultat de la sélection, à partir d'une souche 'apériodique' (composée en fait de nombreux biorythmes autonomes asynchrones), par un vecteur ayant une courte activité nycthémérale, des biorythmes dont la période et la phase correspondent à un cycle d'agressivité.
- Même chez les parasites montrant une périodicité stricte, la subsistance d'une composante circadienne indépendante du système nycthémère est envisagée.
- L'hypothèse d'une origine polynésienne de la filaire de Bancroft, à partir d'un ancêtre austronésien proche de Brugia malayi, est confrontée aux acquisitions récentes sur l'histoire du peuplement du Pacifique.
- Enfin, on envisage que la diéthylcarbamazine serait peut-être un 'déboussoleur temporel', qui perturberait le système de repérage des microfilaires et/ou les 'indicateurs de temps' (synchroniseurs) de l'hôte vertébré.
Ces diverses spéculations montrent que le chapitre sur la périodicité des microfilaires est loin d'être clos, et qu'il mériterait d'être réexaminé à la lumière de progrès spectaculaires récents de la chronobiologie.
The weak and irregular periodicity which characterizes the microfilariae of Wuchereria bancrofti var. pacifica, is tentatively explained as resulting of the superimposition of biorythms differing in phase and in period. This suspected greater genetic diversity could have been preserved by the biting habits of the main vector, Aedes polynesiensis.
Autonomous circadian rythms seem to be extremely stable : through 'folding effect', they are supposed to produce, during more a year, apparent low frecency, cyclical fluctuations.
Following this results, several hypothesis are discussed, in particular about the anteriority of var. pacifica with regard to the nocturnally periodic, 'typical' form, and about the possible chrono-pharmacological effect of diethylcarbamazine.},

A new toxic dinoflagellate found in mixtures of algae and detritus in ciguatera-endemic areas is identified as the most likely cause of ciguatera. Samples contained both fat soluble and water soluble toxins. The former is identified as ciguatoxin, the principal toxin of ciguatera, and the latter is identical with maitotoxin which is associated with herbivorous ciguateric fishes. Realized in French Polynesia among 274 patients with elephantiasis, this survey studied the microfilaremia, the eosinophily, the immunoglobulin titers and the antifilarian antibodies (done by passive hemagglutination) for Wuchereria bancrofti var. pacifica. Patients with elephantiasis seldom have circulating microfilariae in their blood. Hypereosinophily is frequent but rarely high. It
is similar to the one patients with microfilaremia. There is a trend towards neutropenia during lymphangitic crisis that occur on an elephantiasis limb. The IgE titer is clearly increased, the IgG one is lesser elevated. The mean values are identical to those encoutered among microfilariae asymptomatic cariers. On the other hand the serodiagnosis is more frequently positive among elephantiasic patients.},

Twenty-five years after the fight started against Wuchereria bancrofti var. pacifica in French Polynesia and which was based upon mass chimiotherapy by diethylcarbamazine (Banocide), a survey realized in 1975 and 1976 among 274 patients with elephantiasis allows us to give precise details of the local clinical and epidemiological aspects. Since 1950 the number of clinical cases varies along with the parasitological incidence, but with a certain delay which is explained by the late and irreversible elephantiasis characteristics. The overall prevalence is actually low. However the occurrence of new cases, although very rare, is still possible. Among sick patients the lymphangitic crisis frequency whose aspect lead to evoke a bacterial participation, is low. Women are less frequently and less severely involved than men. Localisations are essentially restricted to the lower limbs. Several limbs are involved in approximately 50 0/0 of the cases. The scrotum involvement is rare, the one of the breast is found only once. Among men we can notice a 33 0/0 incidence of associated hydroceles. A past history of chyluria is not rare.},

The suspension of larvae was then introduced under slight pressure into a short sealed section of rat thoracic artery in order to obtain homogeneous distribution of the larvae in the artery walls. Frozen sections of the artery were then suitable for immunofluorescence tests. For the collection of microfilariae, a modification of Sawyer & Weinstein's method was used. Blood from a highly parasitized subject was collected in a citrated bottle, and, after lysis of the red cells with saponin, was centrifuged. Treatment of the sediment with streptolysin then released the living microfilariae, which when taken up in physiological saline solution were ready for use in immunofluorescence tests.},

Cette technique, simple et peu onéreuse, est certainement applicable sous forme de campagnes de masse dans toutes les régions où sévit la filariose lymphatique.The methods in current use for the administration of diethylcarbamazine of lymphatic filariasis are effective against microfilaraemia but require more staff and funds for longer than most countries can afford. The authors describe their findings with regards to the effect of three doses of diethylcarbamazine given at one-year intervals to 120 microfilaria carriers : the proportion of microfilaria carriers in the group fell successively from 100% to 50%, 25% and 12%, the average density of microfilariae in the blood fell from 15 per 20 mm3 of blood to 3, 0.9, and 0.3, and the maximum microfilarial density in the blood fell from 165 to 127, 15, and 11. They conclude that an effective programme of long-term chemotherapy by means of widely spaced doses od diethylcarbamazine (DEC) is feasible and has the advantage of being simple and inexpensive.},
